Dr Aziz Noordin is a family physician at Tampines Family Medicine Clinic in Singapore. His work focuses on community-based care, the management of complex chronic conditions, and strengthening the role of primary care within Singapore’s healthcare system.

He serves as Chairman of the GP+ Co-operative, a not-for-profit organisation that supports healthcare professionals and promotes collaboration within the primary care community.

Dr Aziz has a strong interest in health informatics and digital health, and has been involved in Ministry of Health initiatives related to primary care digital systems and the Healthier SG programme.

He also participates in collaborative care programmes with tertiary institutions and contributes to community initiatives including mental health support and Hepatitis C screening.

Dr Aziz teaches medical students from the Yong Loo Lin School of Medicine (NUS) and the Lee Kong Chian School of Medicine (NTU).
