Ms Janet Lim is Principal Medical Social Worker at Tan Tock Seng Hospital. She has been practising as a social worker in the medical setting for over 15 years. She works primarily with patients and their caregivers at an inpatient rehabilitation setting and supports their re-integration back to the community. She is also involved in projects improving service delivery with the aim of addressing complex psychosocial and health needs. Social Work-Led Joined Up Care (JUC) is one of the collaborative initiative that she is involved in to support vulnerable elderly with health and social complexities, ensuring holistic and wrap-around care as well as improving accessibility to care.
