Dr Sabrina Lee Poay Sian

Dr Sabrina Lee is a Research Fellow with the Clinical Research Unit at NHG Polyclinics, where she leads and supports research efforts focused on chronic disease management and population health across primary care settings. She obtained her Bachelor of Biomedical Science with First Class Honours in Pharmacology from Monash University, Melbourne, and subsequently completed her PhD at the Baker Heart and Diabetes Institute and Alfred Heart Centre at Alfred Hospital. Her doctoral work centred on developing and evaluating strategies to prevent cardiovascular risk factors among high‑risk patients, combining clinical, behavioural, and epidemiological research approaches.

Since joining NHG Polyclinics in 2018, she has continued to build a strong research portfolio in multimorbidity, diabetes, obesity, and cardiovascular disease within the primary care context. Her research interests span chronic disease prevention, optimisation of clinical management pathways, identification of high‑risk patient subgroups, and the interplay of frailty and multimorbidity in ageing populations. She is also a Fellow of the American Heart Association in recognition of her contributions to cardiovascular science. She has authored more than 25 peer‑reviewed publications in international journals and contributes regularly to scientific presentations, interdisciplinary collaborations, and capacity‑building initiatives within NHGP.
