Ms Song Sin Hooi Cecelia
Cecelia is a Nurse Clinician with National University Polyclinics. She has been a Registered Nurse for over 20 years, with most of her experience in primary care. She previously worked in the Care Manager role supporting patients with chronic conditions before progressing into the Nurse Clinician role, where she continues to support Care Managers in their daily practice and development.
Since 2023, Cecelia was involved in the introduction of the Health & Mind Care Manager role within the existing Health & Mind service. She works with the Health & Mind service stakeholders to prepare Care Managers for this role, guiding them in conducting mental health screening at first visits, understanding patient’s concern and backgrounds, and providing follow-up support after treatment is initiated.
Her approach is practical and steady, focusing on growing confidence in nurses and supporting learning at the ground level. She is committed to ensuring patients feel heard, supported and safe when they seek help in the primary care setting.
