Dr Tan is a senior consultant family physician who practises at Pioneer Polyclinic. He is also deputy director of the department of Clinical Services in the National University Polyclinics. Dr Tan leads NUP’s efforts to scale personalised care and support planning, driven by his strong commitment to person centred care and empowering patients to take an active role in managing their health.
