Adjunct Assistant Professor Valerie Teo is a Senior Consultant Family Physician and Head of Kallang Polyclinic at NHG Polyclinics. She plays a key leadership role in advancing innovation across Singapore’s public primary care system, serving as Lead of the NHG Polyclinics Innovation Strategic Workgroup and Clinical Lead at the Centre for Healthcare Innovation @ Population Health Campus (CHI@PHC).
Valerie is passionate about translating ideas into real-world impact and has led and supported multiple system level initiatives at the intersection of digital health, data driven clinical decision support, and integrated care. Her work focuses on translating innovation into real world impact, with an emphasis on scalable, patient centred solutions that improve population health and strengthen care integration.
Her contributions have been recognised through multiple awards, including NHG Innovator of the Year, the NHG Innovate Awards – Excellence in Population Health and the Public Service Transformation Awards – Exemplary Innovation Award, reflecting her commitment to driving meaningful, system level change.
