Abstract

Framed within the World Health Organization’s definition of Primary Health Care as a whole of society, multisectoral approach that integrates health promotion, prevention, treatment, and community empowerment, this session contrasts this broader PHC vision with the more clinical, first contact primary healthcare delivered in the community. This one hour fireside chat brings together perspectives and experiences working in our communities to strengthen primary health care for better population health.
Through these complementary perspectives, the speakers will explore how clinical practice, community nursing and ground partnerships align with WHO defined PHC to support residents where they live, work and age.

Participants will gain insights into how multidisciplinary teams and community collaborations contribute to a more connected, person centred ecosystem that improves population health.
