Abstract

The aims of this symposium is to share on how the allied health professionals and nurses design and plan community programmes for easy access by the residents and yet promote health improvement. The first speaker will share on the journey to adopting Eatwise SG framework in primary and community settings, and the challenges of eating healthier in a country where food is everywhere. This is followed by Community Health Team pharmacist sharing the session on their partnerships with primary care and the active ageing centres to promote medication safety and wellness, and how they debunk common myths and misconceptions in medication use. Lastly life journey of a resident may end off in a familiar home environment. While it seems like a simple wish, the home nurse will share on her experience in developing a generalist palliative care programme for severely frail adults.

From Clinic to Community: Integrating the MOH EatWise Framework in Primary Care and Active Ageing Centres

Singapore's nutrition care delivery has historically relied on fragmented, hospital-centric approaches through Specialist Outpatient Clinics (SOCs), with limited community nutrition services and weak referral pathways creating access barriers and inappropriate care placement.
EatWise SG transforms this landscape through an integrated four-tier system. At the primary care level, primary care nurses are trained to deliver basic nutrition interventions, building frontline capacity for immediate patient support. Established referral pathways enable primary care providers to systematically escalate eligible cases to community dietitians, ensuring appropriate care escalation while reducing unnecessary hospital referrals. EatWise SG curriculum creates standardised, evidence-based care delivery across community organisations. This approach matches patients' risk levels with appropriate care - from community-based support for low-risk conditions to specialist care for complex cases. EatWise SG’s systematic approach enables scalability across healthcare clusters, with standardised training curricula and clear protocols creating a replicable model adaptable to evolving population health needs. By building capabilities in primary care and community organisations, EatWise SG creates a resilient network addressing Singapore's ageing demographics and evolving nutrition challenges, ensuring sustainable care delivery.

From Pill to Partnership: Co-Creating Medication Safety and Wellness in the Community

This presentation explores the transformative role of community care pharmacists within Singapore's evolving healthcare landscape. As Singapore faces rapid demographic shifts with 1 in 4 residents projected to be 65+ by 2030, traditional hospital-centric care models are becoming unsustainable, necessitating a shift towards accessible community-based services.

The session highlights NHG Health's expanded community care pharmacy services, including comprehensive medication reviews, multidisciplinary team collaboration, and innovative skills-based workshops that empower patients and caregivers in medication management. Through the "Fostering Positive Medication Habits" programme, pharmacists deliver hands-on training covering medication label comprehension, medication list creation, and proper pillbox usage.

Building on the MOH Pharmaceutical Care Services framework, the presentation showcases successful GP-pharmacist integration models. Real-world case studies will be shared to illustrate how pharmacists address complex medication challenges.

This collaborative approach represents a paradigm shift from traditional pill-dispensing to comprehensive partnership-based care, ultimately enhancing medication safety and wellness outcomes across Singapore's communities while supporting the nation's Healthier SG initiative.
