Abstract

Value Based Care is the current focus of many health systems internationally with constant availability of new health technologies and treatments that while elevating quality often comes at increased costs which may be unsustainable. Health system funders often asks Primary Care to justify its increasing cost while overlooking the overall value that is created across the health system. We are delighted to have a mix of local and overseas speakers who will share their expert insights on this emerging important topic.

The Primary Care Articulation of Value Based Care

In this presentation Dr Wright will discuss lessons learned from Australian and international experiences with value-based care initiatives. Enablers and challenges to proving the value of primary care will be discussed.

Primary Care and Value: Making a Difference to Patients, Health Systems, and the Nation

The focus on Value is important and timely as Singapore contends with increasing healthcare needs for an aging population. SingHealth Polyclinics is a group of 10 public-sector Primary care centres where multidisciplinary teams practice under one roof. We are also part of SingHealth, an integrated health system with a full range of healthcare services that is also responsible for an assigned population in the East of Singapore. This presentation shows how Value for a chronic condition is managed in Primary Care where our professional relationship with patients can continue over many years, unlike programmes focused on episodes of care, and how efforts in Primary Care integrate with initiatives in the larger health system. When the strategic direction is relying heavily on a pivot to care in the community and an emphasis on prevention, Primary Care is presented with both tremendous challenges and a huge opportunity to make a difference to the health of the nation. Valuing Primary Care enables the Value of Primary Care.

Value Based Care: Effective Primary Care Delivery through Interprofessional Teams

Value based care in primary care is enabled by interprofessional teams working at the top of our license.

This presentation highlights NUP’s care model that incorporates the following three components:

1. Enhanced team based care with 20 minute consult slots, weekly huddles, monthly multi-disciplinary team (MDT) discussions that involve clinicians and allied health professionals
2. Expanded clinical pharmacist led services for medication optimisation, chronic disease management, osteoporosis management, shared care for ischaemic heart disease, etc.
3. Strengthened step down referral pathways that improve the care journey of patients who transition from tertiary to primary care.

Together, these initiatives enhance access, continuity, and patient experience while supporting sustainable, high value care.
